I keep moving my face back from the screen, but still can't seem to zoom out to see the whole picture!
In the first pic, if I'm seeing an upper triple clamp, you're missing the rubber riser bushings and hardware. Stock risers or Hugh's billet riser bushing eliminator kit is the answer.
In the second pic, you may have come up against the annoying and common "hardtail conversion that does not fit any stock axle/spacer combo" problem. You'll have to make or modify spacers to get your chainline perfect, then add whatever washer is required on the outside to get the nut properly tight.
The chain adjusters will take up the gap as there is one on each side. Also Hugh's Handbuilt has aluminum solid riser bushings that are real nice if you don't want to go with the rubber ones from Mikes.
