Leo, I think the diodes or the usual suggestion of replacing the large transistor would fix most of them. The usual suggestion for the transistor is a TIP120. Pete suggested a MJE5742 but it wasn't clear if he'd tried it. They are different enough that between the two one just might work 
Chopit, I wish life was that interesting. I could make up theories, including just coincidence, but it always happened within 300 yds of starting out if it was going to happen, except for the first time it happened. At this point it doesn't matter to me so I've quit thinking about it.
