Wanted - I need one of these clutch washers!

I need the large clutch washer number #4 between the needle bearing and the housing and the spring washer out under the nut. Anyone have these?

Here is what happened to my other one after I torqued the nut down at 58ft lbs. What would cause that?

You didn't have it lined up just right, and it pinched. I've done it before. If you can't find one, I probably have one laying around here somewhere.
